I watched many a perfect autoland in the simulator as an instructor only to see the plane leave the pavement during the turn off the runway because the pilot did not disconnect the auto-throttles and lifted his hand off the throttles to do something else and the power advanced.
Also, there were times the pilot would move the reverse thrust to idle during roll out and keep it at idle reverse during the turn off the runway. If they lifted their hand off the reverse levers to do something, the levers would slowly move into forward thrust and accelerate. I can tell you it was eye opening!   :o

The solution, which is missing in the CS model, is to hit the red disconnect button on the yoke instead of the disconnect bar. The bar only disconnects the autopilots - something very necessary to steer the plane off the center line. Pushing the red button on the yoke disconnects everything!
