If I do a step climb during a flight with the TOD already there, it has no affect other tan to bring it nearer to account for the extra altitude.
I always fly as close to the Optimum altitude, shown on the VNAV CRZ Page, as I can, so I only do a step climb when the FMS tells me to do it NOW. ;)
