Years ago when canada switched from british measures to metric. An air Canada pilot on a B767-200 got his fuel loaded in lbs, he though it was kg. Half way to destination, he ran out of fuel at FL400 around Winnipeg Man,  being a former Air Force pilot, he remembered an old training field now deserted.called Gimli  He glided his plane there and landed succesfully. Air Canada fired the pilot. The Canadian Gvt gave him the highest decoration for saving his pax.

I've also seen that one on National Geographic - Air Crash Investigation. The last time being about a month or so ago.

National Geographic runs 2 episodes of Air Crash Investigation a night, from Monday to Friday. So I get to watch them a lot. ;D
