I nearly looked over it but there must be a mistake in your weight calculatiion.

With an "empty weight" (BOW) of 115.000 lbs in FS2004 and an under average passenger/cargo load of 18.900 lbs, the ZFW is already at 133.900 lbs, so that would mean you loaded only about 6.000 lbs of fuel - you could take the bus instead  :D (the 757 wouldn´t go far with that low fuel)

Now you said the GW (ZFW + trip fuel + alternate fuel + reserve fuel) is 140.000 lbs. So I guess you accidentally entered the correct numbers for the ZFW into the line for the Gross Weight (which is calculated automatically by the fmc once the ZFW is entered correctly). No wonder the poor FMC went havoc during the approach speed calculatiion  :)
