When P3D is out of focus all CS custom sounds, such as the background noise in the cockpit, stop playing and only the P3D internal sounds continue to play.

Since it is a native P3Dv4 feature to continue playing the sounds when the simulator is not in focus I believe this should be working with your custom sounds, too.
It's really an immersion killer if I open TOPCAT to calculate my takeoff/landing data and suddenly I loose all my sounds.

Not a bug but might work as a suggestion.

Just a minor matter, but the cabin call chime/Selcal sound only appears to work on the ground (or else it's too quiet to hear when airborne, but I've listened carefully, and it doesn't appear to work in the air)

You can adjust a volume of any system sound in:
...\Lockheed Martin\Prepar3D v4\Captain_Sim\757-III\sound.ini
Use Volume=X.XX (anything from 0.00 to 1.00)
